The Palliative Care Nurse Practitioner is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Palliative Care Nurse Practitioner provides palliative care for patients facing chronic, complex, and /or life-threatening conditions at various locations within the flow of patient care. S/he provides assessments, disease and treatment education, and symptom management to meet the needs of palliative care patients and their families. The Palliative Care Nurse Practitioner helps the patient and family maintain their maximum level of comfort and coping. S/he provides care and support in collaboration with the PCP and specialists and ensures compliance with practice standards and codes. S/he must collaborate appropriately with the assigned Compassus Medical Director and flex their schedule to meet agency needs. Position Specific Responsibilities include performing routine and urgent assessments, availability Monday – Friday, 8a-5p local time, minimum 100 visits per month, carrying a minimum patient census load of 80 patients, triaging consults by patient acuity, bedside palliative care with patient/family education, after hours and weekend on-call requirements, ordering and interpreting lab and radiology tests, prescribing medications including controlled substances, ordering treatments and durable medical equipment, consulting with palliative care physician or designees, assisting in care coordination, maintaining accurate patient records, complying with laws and regulations, completing billing and coding, providing training and education, defining goals for professional growth, adhering to confidentiality, assisting patients and families with treatment goals, collaborating with interdisciplinary team, educating patients and caregivers, supervising nurse trainees and other trainees, regular regional travel within 60-mile radius, and other duties as assigned. Education and Experience required: Master’s degree in Nursing, minimum five years nursing experience preferred in oncology, hospice, geriatrics, intensive care. Skills include mathematical, language, communication, organizational, time management, flexibility, travel ability, and confidentiality. Certifications required: Active CNS or Nurse Practitioner license in state of employment, Nurse Practitioner certification in Adult, Family, Acute, or Geriatric, active NPI number, DEA certification if applicable, Board Certification in Hospice and Palliative Care preferred. State specific requirements include collaborative physician agreement except Iowa. Physical demands include standing, walking, manual dexterity, lifting up to 50 pounds, visual acuity, exposure to bodily fluids and infectious diseases, use of medical equipment.
